Documents Required at GPAT 2026 counselling
Candidates who qualify for the GPAT 2026 examination must submit the following documents during the M.Pharm admission and counselling process conducted by participating institutes or state counselling authorities.
- GPAT 2026 Scorecard / Rank Card issued by the National Board of Examinations in Medical Sciences (NBEMS)
- GPAT 2026 Admit Card
- B.Pharm Degree Certificate or Provisional Passing Certificate
- B.Pharm Mark Sheets / Semester-wise Academic Transcripts
- Class 12 (HSC) Marksheet and Passing Certificate
- Class 10 Certificate (for date of birth verification)
- Valid Photo Identity Proof (Aadhaar Card / PAN Card / Passport / Driving Licence / Voter ID)
- Category Certificate (SC / ST / OBC / EWS), if applicable
- PwD / Disability Certificate, if applicable
- Migration Certificate (if required by the admitting institute)
- Transfer Certificate (TC) from the last attended institution
- Domicile Certificate (for state quota admissions, if applicable)
- Recent Passport-size Photographs
Candidates must carry original documents along with multiple self-attested photocopies for verification. After successful document verification and completion of admission formalities, candidates will be allotted M.Pharm seats based on GPAT merit, institute cut-off, and counselling guidelines.
